Direct Access Physical Therapy in Cherry Hill, NJ: Start Without a Referral

What Direct Access Physical Therapy Means

Direct access means a licensed physical therapist can evaluate and treat you without a referral or prescription from another health care professional. You go straight to the clinician who treats the injury.

The American Physical Therapy Association (APTA) is the profession’s national body. It defines direct access as evaluation and treatment “without the need for an order or referral from any other health care professional.” Every state has now removed all or some of those referral requirements.

Direct access isn’t a discount, a program or a package. It’s a legal right. It removes the doctor’s appointment you used to need before you could book the appointment you actually wanted.

Do You Need a Referral for Physical Therapy in Cherry Hill?

No. You don’t need a referral for physical therapy in Cherry Hill or anywhere else in New Jersey. Public Law 2003, chapter 18 removed the prescription requirement, so a licensed New Jersey physical therapist can evaluate and treat you directly.

Two words cause most of the confusion here. A referral is something state licensing law may require before treatment starts. Coverage is what your insurance plan decides to pay for afterward. What New Jersey Law Asks of Your Cherry Hill Physical Therapist

Once direct access care begins, New Jersey law gives your physical therapist three duties, and all three are there to protect you. They’re set out in the State Board of Physical Therapy Examiners rules at N.J.A.C. 13:39A-2.5, under the authority of N.J.S.A. 45:9-37.18.

  1. Inform your provider: Within 30 days of your first treatment, your physical therapist must tell your licensed health care professional of record about your plan of care. If you don’t have a provider on record, your physical therapist will recommend that you see a licensed health care professional of your choice.
  2. Refer you if progress stalls: If you haven’t made reasonable progress within 30 days of your first treatment, your physical therapist must refer you to a licensed health care professional.
  3. Refer you when PT isn’t right: If there’s reason to believe physical therapy is contraindicated, or you have symptoms that need care outside a physical therapist’s scope of practice, your physical therapist must refer you. Under N.J.S.A. 45:9-37.34c, failing to refer in that situation counts as unlawful practice of physical therapy.

The first two duties are often called the New Jersey 30-day rule. It isn’t a cutoff, an expiration date or a visit cap, and nobody stops your care on day 30.

In practice, your physical therapist sends your plan of care to the provider on your record. If you aren’t improving, you’re pointed toward the right workup rather than given another month of the same treatment.

Rehabletics reassesses your progress well before day 30. Most athletes notice a difference within their first two or three sessions.

State Law and Your Insurance Plan Are Two Different Rules

State law decides whether you can be treated. Your insurance plan decides whether it pays. The two questions have separate answers, and confusing them is what keeps people waiting.

Rehabletics is an out-of-network sports physical therapy clinic in Cherry Hill. Your insurance plan doesn’t decide how long each session runs, how many sessions you get or when you’re finished. Your clinician makes those calls.

Your plan may still require a physician’s prescription before it reimburses an out-of-network claim you submit. Ask your insurer directly, because the answer varies by plan. One exception: auto insurance. If your care is billed to New Jersey personal injury protection (PIP) benefits, state law does require a referral. Under N.J.S.A. 39:6A-4, physical therapy isn’t reimbursable under PIP coverage without a referral from a licensed physician, dentist, podiatrist or chiropractor. When You Should See a Physician First

Direct access is meant for musculoskeletal injuries, not medical emergencies. Some symptoms need a physician, an urgent care center or an emergency room before you see any physical therapist.

See your doctor or go to an urgent care in the Cherry Hill area first if you have:

  • A joint or bone that can’t bear weight after an injury, or an obvious deformity (both can be signs of a fracture)
  • New or worsening numbness, tingling or weakness, especially in both legs or both arms
  • Loss of bladder or bowel control along with back pain
  • Fever, chills, night sweats or unexplained weight loss along with your pain
  • Chest pain, shortness of breath, or pain that spreads into your jaw or left arm
  • Severe pain that wakes you every night and doesn’t change with position or movement
  • A head injury with confusion, vomiting, vision changes or a headache that keeps getting worse

High school and college athletes: A trainer’s clearance, a coach’s sign-off and a physician’s referral are three different things.
